3. Minimum of 80 Clinical Rotation In Physician's Office or Urgent Care Center
4. Or A Minimum of 1 Year of MA Experience in Physician's Office

6. Successful Completion of Certification Exam
CLINICAL RESEARCH ASSISTANT: EDUCATION
1. Completion of an Approved Phlebotomy/EKG/I.V Program
2. Minimum of 80 Hours Lecture
3. Minimum of 80 Clinical Rotation In Medical Lab or Research Center
4. Minimum of 50 Successful Venipunctures
5. High School Diploma or G.E.D
6. Successful Completion of Certification Exam
PATIENT CARE TECHNICIAN: EDUCATION
1. Completion of an Approved Patient Care Technician Program
2. Minimum of 80 Hours Lecture
3. Minimum of 80 Clinical Rotation In Phlebotomy
4. Must Be Certified Nursing Assistant
5. High School Diploma or G.E.D
6. Successful Completion of Certification Exam
EKG TECHNICIAN: EDUCATION
1. Completion of an Approved EKG Program
2. Minimum of 60 Hours Lecture/Practicum or Clinical
3. High School Diploma or G.E.D
4. Successful Completion of Certification Exam
CLINICAL RESEARCH ASSISTANT: EXPERIENCE:
1. Completion 1 year full-time employment in Clinical Research Facility
2. Research Facility Must perform Phlebotomy/EKG/I.V
3. Letter of Confirmation From Employer
4. High School Diploma or G.E.D
5. Successful Completion of Certification Exam
PHLEBOTOMY TECHNICIAN: EDUCATION
1. Completion of an Approved Phlebotomy Program
2. Minimum of 40 Hours Lecture/lab
3. Minimum of 50 Successful Venipunctures in class or clinical site
4. Minimum of 10 Skin Punctures (Capillaries)
5. High School Diploma or G.E.D
6. Successful Completion of Certification Exam
PHLEBOTOMY: EXPERIENCE
1. A minimum of 1 year full-time employment as a phlebotomist or related field
2. Documentation of Employment

MEDICAL LABORATORY ASSISTANT: EDUCATION
1. Completion of an Approved MLA Program
2. Minimum of 40 Hours Lecture

MEDICAL LABORATORY ASSISTANT: EXPERIENCE
1. I year of full time employment in related field
2. Letter from employer verifying employment

HEALTH CARE INSTRUCTOR: EDUCATION & EXPERIENCE
1. Bachelor Degree in Education with a Concentration in Natural Science (Biology or Chemistry or Physics) or 1 Year of Experience in the Health care Industry

3. No Exam Required
